Most of the buses seem to have tapes, not live persons. Maybe there are exceptions. Ten years ago my family wanted to do the bus tour, got the headsets, and were asleep in 10 minutes. Three years ago my parents loved it.

The BEST English speaking tour guides in Paris are not on a bus tour (I too think the buses are all recorded to appeal to the masses) but rather on a walking tour. If you really want a good tour for a Paris neighbourhood check out Paris Walks - http://www.paris-walks.com

Then if you like Architecture, you have to stop by the Cathedrale of Notre-Dame !!! Riverwalk of the Seine, Promenade in Montmartre. Top of the Eiffel Tower, Triumph Arc, Grande Arche, Montparnasse Tower, The Champs Elysées and Pigalle by night...

If you're in Paris around the 14th July, try to get a good spot for early in the morning around the Champs Elysees for the traditionnal march past, or as high as you can get around 10.50 for the fly-past. In 2008 I was lucky enough to be at a friends place (7th floor) and got a good view of the 'planes moving down the Champs Elysees and around down to the south of Paris.
I brought good walking shoes. There are lots of cobblestones that are hard on you if you wear heels. I also wore jeans and black pants. Basic neutral colors seem best if you don't want to stand out. In the winter bring a warm coat and wear a scarf. An umbrella is a must. Bring basic headache and cold medicine. I got a cold while there and had trouble describing my symptoms to the pharmacist. All I knew how to say was "je suis malade". I couldn't tell her I had a sore throat. The pharmacies are clearly marked with a neon green cross. A good camera is a must. Bring plenty of film and if you use a lithium battery, make sure to bring and extra!
