Walking around old town
Toledo old town was built on hills and thus it takes lots a walking up and down. So the map may not save you enough time and you shouldn't be on a tight schedule if you wanna see all parts of the old town properly.

I would like to take a high-speed AVE train from Madrid to Toledo and would love to join an organized tour of the city (need it to be in English since my Spanish is not that great).
Any suggestions? Are there any reputable companies organizing these kinds of tours?

I don't know about tour companies, but it is very easy to catch the train on your own and do your own tour. Toledo is not large and a lot of what you will want to see is in the center. There is also a tram from the center square that loops the city and is well worth doing.

My main concern is that I won't know what I am looking at (that is, I would like someone to tell me about the buildings, the history behind them, etc.).

If you follow all the embedded links, it shows photos of many of the places. You could print some off if you'd like ... and there's the tour. ;=)
Toledo is an active tourist area. There are many information signs in English and tourist information booths with maps, etc. in English.
I agree with the previous posters. You better download the information from the internet(I am sure you may find tons), then even print what you consideer more interesting...and you don´t need a company.
I am sure that if you have doubts, some vters will help you.
Only problem I see is if you have time enough to "study"your trip.

Its a fascinating place to visit and part of the charm is getting yourself lost in the tiny streets - nothing seems a straight line in Toledo. We happened to bounce a tour in the old synagogue and thinking it was part of the same place sort of tagged along to other buildings in the area until the guide mentioned lunch - LOL. We got to see more than we bargined for.
Agree with other posters, its a small city and get a guide book or research on the net and then "get lost" - LOL. I believe Toledo is worth staying at least overnight, but 2 is better. The train to/from Madrid is only 45 minutes and the Toledo rail station itself should be heritage listed along with the rest of the old city.

After that ... We were so thrilled (hahaha) that we urgently needed a bathroom... I just realized that I had not seen a WC anywere in Toledo... So, if you need one, there is this yellow building next to the wood house... It´s kind of hided and it doesnt say it is a WC... Also it is very dirty, but... When you have to go... You have to go...
The Old Town isn't very big, but hides lots of things that even lots of Toledo inhabitants may not know. Every time you go there you find a different street, bar or restaurant, so it's cool to try to lose yourself in this street labyrinth in order to find new places.
My last discovery was an Arabic restaurant between Comercio St and Lucio's St, in a narrow alley which joins these two streets.
It is not very big, but big enough and it's decorated with Muslim adornments. It offers very cheap Arabic food which I love and you can order a water pipe if you are over 18. You can also order take-away food and they serve you very fast. I think that their Kebabs are the best in Toledo.
It is near the center of the Old Town, so you can stop, have lunch and continue your visit around Toledo.
Look for some stairs... If you walk down you will get to the riverside, and will able to have a whole new and also great perspective...
