I am taking my husband to Quebec in May for his birthday. We would like to see Old Quebec City and some of the nearby countryside. Can anyone recommend a small inn, b and b or boutique hotel that is a little outside the city, but near to public transportation so we can get to the city without a car, and maybe do some hiking/walking/biking in the country?I'm hoping to find a place on the side west of the city.

It is not clear from your message if you are driving to Quebec City or taking public transportation such as train or plane).
As you enter the city (from the west), there are tons of hotels (including boutique (e.g. Alt hotel)) where you could park your car for free and get into the old town by bus. Howver public transportation is not the most efficient and the cost of two tickets round trip will offset the cost of parking your car in the old town. It is much nicer to stay directly into the old town and walk around at your leisure. Many hotels will offer parking for a small fee (around 10$-15$ /day). Getting in and out of the old city by car is not that big of a problem and you can always use you car for a day in the country (e.g. Ile d'Orléans, Charlevoix etc). Besides, the old town has a lot more B&B's and boutique hotels that would suit your purposes.
I usually recommend le Manoir d'Auteuil (http://www.manoirdauteuil.com/) for a real B&B atmosphere and great location inside the walls or Le Priori (http://www.hotellepriori.com/fr/) for a boutique hotel.

Hi! Unfortunately, as soon as you get a bit outside of the city, there is little to no public transportation available. Your options are to stay in the Old City, as the previous poster mentionned, or if you'd rather stay in the countryside and don't mind driving to the Old City, then you could look at staying in Duchesnay (http://www.sepaq.com/ct/duc or http://www.cjduchesnay.ca). It's about 30 min west of Quebec City and there's plenty of hiking/walking/biking to be done in the area. Another option could be to stay at the beautiful First Nations Hotel (http://www.hotelpremieresnations.ca), located about 20 min north of Quebec City in Wendake. I've also stayed in lac Delage before (http://www.lacdelage.com), not too far from the city but in a nice natural setting with many outdoor activities offered. Hope this helps!
