Perurail has a train that does the route. You can also fly from Cusco to Julica (a town just outside of Puno).
When we went (4 of us) we arranged for van and driver with a local(Cusco)tour operator, but really can't break out the cost, as there were several other things buried in it (but it was less expensive than 4 train / plane tickets).
Travel Tips for Puno
Puno: One of the highest poverty rates in Peru!
The Puno area of Peru has one of the highest poverty rates in the country. I didn't know this fact until after the guide said it during our tour.
If you are ever in this area of Peru, here are some tips to be a socially responsible tourist:
1.) Buy local goods (e.g., buy from street vendors, eat street foods as much as you can, etc.)
2.) Be more generous with your tips (give at least 35-60% tip for any service rendered)
3.) Patronize local tour operators/agencies
4.) Bring something you can donate to a social service agency
5.) Refer possible visitors to this area of Peru

These very interesting pre-Inca funeral towers are called chullpas. Most of these towers were built by the Colla (Aymara) in the 15th Century, for the burial of their important people. The chullpas are round, made with large stones, and are larger at the top than the bottom. One of the towers was white, and it was for women.
A long path loops back and forth from the parking lot to the top of the hill, going past the various towers. It is not a difficult walk.
There are some Inca ruins here also. The Intiwatana (Capture the Sun) was a solar calendar. There are 3 stair steps into it, representing the snake, puma and condor. Compasses don’t work within the circle due to magnetic rock.
Sillustani is located beside Umayo Lake, 30 km (18.7 miles) N of Puno. Take the highway toward Juliaca, and then turn onto a secondary road going west.

I´m repeating that tip, because it´s great:
Don´t go by bus, better go by train to Cusco. Great adventure. Buy your ticket a day before at the station, preferable third class, it´s cheaper and much more interesting!!
We went there on our way back from the two day trip to the Isla Amantani (we spent one night there with the Locals).
And there on the island we just took a walk.
Interesting is always to hear the stories of how the color or shape of the heads/caps show if someone is single or married - doesn´t matter if man or women.
Lake Titikaka: Taquile Island
The second day, I took a tour to vast the Peruvian side of Lake Titikaka. The second island we visited was Taquile, which has a cultural treasure in itself (sadly being destroyed by tourism). Here, the people dress in traditional clothes (even man, which is a rarity in South America).
