Al Fishawi's claims to have never closed it's doors since it first opened in 1773! It's open day and night, and is a really atmospheric place to have a glass of mint tea, and possibly smoke a shisha (it's not just for men, at least here; I saw plenty of women, including Egyptian women, smoking shishas).
You order from the passing waiters, and pay at the end - careful of your pens though, they like to grab them and hold them for ransom as a joke! There are also a lot of people hawking goods, but they go away if you say no. Lots of cats too - making friends can be a bad idea as they'll never go away!
I think 2 teas and a shisha came to less than 20LE.

Most famous cafe in Khan EL Khalil....it gained it`s reputation from all the Artists and writers who used to come and sit here.
This cafe has not closed for a day since the past 200 years!
Once I went with my cousins..one of them plays the (darabokah) it`s like a drum...the other plays the guitar....we had a great night singing and drinking tea.
Along came this old man and played on the flute.
Favorite Dish: We had only tea and smoked the shisha.

ask ANY VENDOR in Khan-- they will point you to this place.
To take a break while shopping at Khan al-Khalili, this is a great option. Famously known for being the oldest coffee house in Egypt, this place is open 24/7 and hasn't closed once during the last 200 years (except during Ramadan period). Tourist prices, but great sheesha and bustling atmosphere.
Though be warned-- you WILL have to fend off touts while you're taking this coffee break. They're just everywhere in Khan.
Favorite Dish: I personally really enjoy the apple and banana sheeshas. Give them a try if you've never smoked sheesha before! :)

We got a chance to visit this old teahouse that is truly a part of the egyptian culture and heritage, AND it was on one of the vibrant nights of Ramada. We went there after spending hours walking around Khan El-Khalili and shopping till be dropped...and we dropped in the perfect place ;-)
This teahouse is the oldest teahouse in Cairo. It is in the heart of the Khan el-Khalili Bazar and has been open since 1773. The name 'Fishawi' is actually the last name of the family that runs it. Seat yourself on a wooden stool in this place, order a shisha and some of that dark dark egyptian tea, and enjoy looking around at the wood-work, the cracked mirrors, and beautiful furniture.
Favorite Dish: - Hubbly-Bubbly...or as they say in arabic/egyptian shisha: there are flavors which you can smoke as well (apple, roses, grapes, etc..)

Not only is the food yummy and cheap, but this is a historical joint that is known to have been a favourite hangout of all the hot shot poets and politicians and scholars of the 1920's-1960's
You'll occassionaly see fun loving guys singing and debating, or playing chess
