I've been reading a lot of the night life reviews on here and it seems everyone has developed the same opinions or they just don't know about all the other great clubs out and about Tokyo.
Given, some of them take an insider tip...
FERIA (or LEX (same sort of thing.. but I prefer Feria) : This was the very first club I set foot in. It's not a large club, reasonable prices for admission and drinks, DJ usually spins the same tunes played in clubs around NYC.
What's different about this place is the "scenery". It is completely littered with models. They eat and drink for free so you'll see them there almost every night. It's doubtful you'll be able to make into VIP (a large balcony above the rest of the club) to shmooze with them, but the ground floor is just as packed and full of crazy party people. Best time to come is Friday or Saturday night. Oh, and if you want to attempt to get in for free, tell the bouncers Dio is expecting you.
ULTRA CLUB : This is not a place many forgeiners know about. It's not in any of the guide books I've ever read. I would have never step foot in there if it wasn't for a good Japanese friend of mine. The place is very stylish, throws exuberant theme parties on certain holidays and plays some great house music.
Velour : Probably my favorite. The interior of the club is absolutely stunning. Check out the web site of the place. A lot of Tokyo's upscale fashion crowd hang out there, as well as some very important business people. If your lucky enough to get into VIP, it will be an experience you will not very soon forget.
La Fabrique : I came here for my birthday. The music was fantastic, drinks you wouldn't find elsewhere and the owner is a complete sweet heart. They have some great theme parties. BOOGIE NIGHTS is my favorite.
Amex : If you want to be the only white person at a club, come here. It was like stepping into another world. You'll probably need a guide that's been there before to find it (it's slightly outside of Shibuya.. down a lot of dark alleys). It is an experience like no other. A lot of Tokyo's very eclectic and hip crowd hang out there. You'll see a lot of people dress in Harajuku or drag. The interior is what I would call "Twisted Alice In Wonderland". There's a huge dance floor and House music shakes the walls to their core.
Check them out. There will never be a dull moment.
Dress Code: Feria/Lex: casual, but if your hoping to get a model to talk to you, it wouldn't hurt to make an effort.
UltraClub: It's laidback and minimalist, but don't show up in shorts or something.
Velour: It's very chic and sophisticated. Wear your best Fendi and Dior ensembles to fit in.
La Fabrique: quite casual.. or just some cool, colorful club wear
Amex: the crazier, the better. You really can't go wrong here.

Address: Just look up the clubs by name on google.
These guys usually throw some good hip hop/r&b parties in central Tokyo or farther afield in Fussa or Yokohama near the US military bases...
You'll find a mix of Japanese and American people from 18 to mid-30s and lots of southern style hip hop too. Unfortunately they don't play too much reggae.
Dress Code: You can dress up in a suit or dress down with jeans unless otherwise specified on their flyers. Women usually dress in less!

Contrary to my findings of music populatrity in Tokyo - there are a LOT of R'n'B clubs in tokyo and they are mostly free.
Roppongi, obviously, is amazing and the cost of getting back by taxi can be astronomical. It cost me 3500 Yen to get to my hotel (I made the decision to enjoy the night). There are hip-hop clubs all along Roppongi.
Shinjuku OR Shibuhaya has Gaspanic which is a free r'n'b club.
You obviously have to buy drinks at these free clubs.
There are tooo many pimps and pros hanging around Roppongi though.
You have to remember though that trains stop before midnight and the cost of
Avoid clubs like Harlem - they have to much 'tude and are expensive.
Dress Code: Just dress sensibly (even Jeans and Sneakers are ok) ...
