From the airport in Montego Bay, like most people we got on a shuttle that was included in our vacation package. After going through customs, you pass into an area that is filled with all of the different tour company representatives and you should be able to spot yours easily. From there they accumulate all of the people that are going to resorts in the area you have chosen, you give your luggage to a porter even if you can easily carry it yourself (Jamaica is an extremely tip based economy) and then get on the bus.
From the airport it's about an hour to get to Runaway Bay, add another 30-45 minutes to Ocho Rios. Our trip took slightly longer as our driver stopped a couple of times to pick things up and annoyingly just 10 minutes away from our resort for a beer break. Really, people can't make it 1 1/2 hours for a beer and bathroom stop?
The driver will remind you to tip him when getting off the bus but no one from our resort did, we were all annoyed that he stopped so close to our resort.

If you wish to go sightseeing, it's very easy to hire a cab. Ask the Front Desk clerk or tour desk at your hotel to locate a reputable taxi driver. There is sure to be one in your area.
We took a cab from Hedonism III to Dunn's River Falls. It was $25 roundtrip. While it would have been cheaper to take the hotel tour to Dunns River, we liked this better as we could come & go when we wished, rather than be part of the crowd.
Taxi fares are negotiable, and you should ask upfront before you enter the cab. Pay the driver AFTER you've done all your traveling. For instance, we paid our driver after we came back to our hotel. This way, you know they'll wait on you!

Helpfulness
Most of the resorts offer shuttle transfers between Montego Bay & the resorts. These range from small vans to big city buses. The price is normally included in the cost of your resort package.
The trip takes approximately 1 1/2 hours. The bus will stop about halfway at a bar/souvenier shop. Here you can purchase soft drinks, alcoholic beverages, postcards, and arts & crafts. Expect to stay for about 30 minutes before venturing on to your destination.

The Driver's personality was GREAT! His driving was too fast for me to take the pictures I wanted to before we got to the resort, but it ended safely. It was air conditioned, too, so that was nice.
