The easiest and the quickiest way to get to Malpensa airport, and vice versa, is by the city train express. This is Stazione Cadorna, the train station from where the train starts to Malpensa and vice versa, where the train is coming from the airport. The station is situated on Piazza Cadorna, you can't miss it coz there is a giant needle and thread monument coloured in various bright colours.

For people who leave Italy from Airport Malpensa and have to get tax refund from your handed-luggage, you'd better watch out the police cause he lied to us about getting the tax refund in Zurich. If you transfer in Swiss which is not member of EU, make sure that you can only get tax-refund in area B but the boarding gate is in area A. It's really unbelievable that the POLICE told us Swiss is one of EU!!!!!

When you fly to Milano, most probably you'll land to the biggest airport - Malpensa, cca. 35 km away from Milan. Do not miss the spectacular view of the Alps stemming from the clouds! If you're lucky, you'll be landing at sunset/dusk, and the images you'll see are simply surreal... The chains of Alps covered in snow rising up from the clouds, embracing the sky and the crimson layer of the setting sun...

Malpensa is not exactly the best airport I've seen. The modern structure is quite impressive, but it's about 1h from downtown and the airport suffers from chronic air traffic control problems. They even lost my luggage once without a connection (evidently an hour wasn't enough for the bags to reach the plane from check-in...)

I recommend the flights to Bergamo with Ryanair for trips to Milan.
It will often be the cheapest alternative for you and Milan central train station is only a short and inexpensive expressbus ride away.

In Milan, there are two airports, one is Malpensa Airports, located 52km from city centre and the other one is Linate, located only 6km. Most of internation flight will landing or departure from Malpensa, but some of them also from Linate. SO could considered both is international airport.
From/to MALPENSA airport;
1. Train; Remember that Malpensa Express only departure from-to Cadorna-Ferrovie Nord (every 30 minutes and took 40min; charge can be 11 euro). This train station is NOT Milan Centrale. To get there, take Metro Line 2 and 3 only, and stop at Cadorna.
2. MALPENSA bus departure from Milan Centrale (main train station) and take around 60mins. Charge can be 5.50 euro only, departure every 30min.
3. Malpensa Shuttle, also from Milan Centrale (every 20 mins; 5 euro)
4. Taxis; the most expensive can cost you up to 75 euro, and only white taxi! No other taxi. Airport taxi should display 'taxi autorizzato per il servizio aeroportuale lombardo' on the screen.
From/to LINATE airport;
1. The better is taxi and cost only 20 - 25 euro. This WHITE taxi line up outside a terminal (easy to see it).
2. ATM bus No 73 (every 20mins and take 25mins). This bus will go to Piazza San Babila (ticket cost only 1 euro).
3. Coach to Milan Centrale (every 30 mins) cost only 2 euro and take 25 mins.

For those traveling to Milan, you should know that there are two airports serving the city. Linate and Malpensa. Malpensa is the newest of the two airports, and it is the one I arrived in. Completed in 1998, it is a thoroughly modern airport with shops, restaurants, duty-free stores, dry cleaners and everything you could want or need in an airport. It also has excellent transportation by train or bus available for travel into the heart of Milan which is 40 minutes to 1 hr away depending on your mode of transportation. The city of Como is also roughly 30 to 45 minutes away by train.
Linate airport has the advantage of being only 7km from downtown Milan. Linate is the airport probably preferred by those whose only or primary destination is Milan, and therefore may host more intra-Italy commuters, and/or less expensive airlines.
I am not totally certain about Linate, but I know that Malpensa airport has hotels nearby which have courtesy shuttles which will pick you up or drop you off at the airport for free or the nominal cost for the driver's tip. However, for very early flight departures, make sure that your hotel's driver is available or pre-book a taxi for the time you desire.

There are three bigger airports around Milan. The biggest one is Malpensa, where aeroplanes from all over the world comes and goes.
There are two terminals, with the trains and busses going to the international one. If you're going to take a national flight there will be shuttle busses that takes you there.
From Malpensa you can either take the bus (which starts right outside the airport). There are a number of different companies, so you will almost always find a bus that goes away quickly. Prices range from 4,50 euro to 5,50 euro for a one-way-trip. The busses stops outside the central station, "Stazione centrale" in Milan. The trip takes one hour.
You can also take the "Malpensa Express" which is the train. Costs 11 euro one-way, but goes 15 minutes faster than the bus. The train stops at the second biggest train station in Milan, Cadorna station.
I would guess that a taxi from Malpensa to somewhere around 70-90 euro. Not worth it, if you ask me. Better then to take the bus/train to the city center, and from there a taxi to your hotel if you're unsure where it is.
The second airport is Linate, which is quite much smaller. It's also much closer to the city center. A taxi goes for 15-20 euro one way. There are also busses that goes to the central station, or local busses that goes to San Babila, the bus stop next to the Duomo. That ticket costs 1 euro. For a shuttle bus I would think the ticket price is about 3-5 euro.
If you go with Ryanair (Among the destinations Barcelona, Paris, London, Frankfurt) you will come to the airport outside Bergamo. It's as close to Milan centre as Malpensa, so there is no difference. From Orario De Serio you take the bus to either Bergamo or Milan. Takes an hour to Milan, and the trip costs about 7-8 euro if I'm not mistaken.
