Milan Malpensa Airport has two terminals
Terminal 1 is the larger of the two terminals. It handles most domestic and international flights.
Terminal 2 is mostly for charter flights plus a few international services.
To get to Malpensa by Car :
Access roads from Milan are A8, A26. A new road is planned linking the A4 Milan-Turin highway to Milan's western ring road.
Journey time is around 45-50 minutes but can be longer at peak times.

Malpensa International Airport (MXP) is Milan's largest airport, located in the northern part of the city, in Busto Arsizio.
Designed by the famous Ettore Sottsass, Malpensa Airport is very modern and very well connected to the downtown through Malpensa Express railway service from Cadorna Station and Malpensa Shuttle Bus from the Central Station.
Milan has also other two smaller airports: Linate Airport (LIN) used for European and domestic flights and Orio al Serio Airport (BGY), located close to Bergamo and used by low-costs companies.

This airport is a much used transit point for flying with Al Italia.
The airport leave the following to be desired:
1) Everyone has to pass through a small security check, with a resulting scrum is 2 aircraft arrive at once
2) Getting into the City is not quick
3) The services in the airport are not the greatest (a few posh shops, but short of some good cafes)
Too many people for the facilities.
Also, there is a tendency for many delays due to fog!

It's very important remembering that Milan has two airports: Malpensa (MXP) that is the main airport and also has intercontinental flights and the Linate (LIN) airport that serves domestic flights and some European links.
To go to Milan from the Malpensa airport you have 3 ways.
1)by bus: The bus leaves from Air Terminal piazza Luigi di Savoia and in 50 min arrives to the airport and by the coach bus Malpensa Shuttle http://www.malpensashuttle.it/e-default.htm.
2) By the Northern Railways:
“Malpensa Express” train [http://www.ferrovienord.it/webmxp/]http://www.ferrovienord.it/webmxp/ . It takes 40 min, its first bus leaves at 4,20am from cadorna, the last one at 11,27pm. From Malpensa at 5,53am the first one, at 1,30pm the last one
3) By Statal Railways:

Linate is linked to Milan by bus. The bus 73 ATM starts from the large square in front of the airport to the center, in Saint Babila's square, takes 25 minutes with trips every 10 min.;the first bus leaves Linate at 06,05am and the last one at 0,55pm. From S.Babila the first leaves at 5,35am and the last 0,35pm.
The service "Autobus Starfly" - tel. 02 717106 leaves from Luigi di Savoia square to go to Linate with trips every 3min(from 5,40am to 9,35pm) and takes around 20 minutes.
Linate and Malpensa are moreover connected to each other by a shuttle service provided by Air Pullman.
At 45 km from Milan you will find also Orio al Serio (BGY) international airport. It is Italy's first low-cost airport with many flights for Italian and European destinations. From the airport there are 30 daily coach service runs to Milan Central Station provided by "Autostradale" and "Locatelli Air Pullman".

I flew into Milan, or actually Bergamo (40 minutes north-east of Milan) from Larvik, Norway on Ryanair. It is a great and cheap way to fly around Europe, and I think that this was the cheapest way of getting from Norway to Italy.
The seats on Ryanair aren't bad, but any trip longer than 2-2.5 hours and you might start to feel a bit stiff in your seat. If you want something to snack on when flying you have to either bring your own food or pay for snacks on board.
With Ryanair from milan you can fly to: Norway, Sweden, Austria, Belgium, the UK, Ireland, Germany, and Spain.
Once at the airport there is a connecting bus that will take you to the Milan Central (Train) Station. You can buy tickets off the Ryanair website (under 'destinations' and then 'Milan'), but must be more than 24hrs before your flight. You can also buy them onboard the flight or when you get into the airport at the desk in the main hall.

Milan is served by three airports:
1) Malpensa - located way outside Milan, reachable by bus, airport express train or of course by taxi. Give yourself plenty of time to get here if you are coming by road (esp. during rush hour and one Friday and Sunday evenings) as you will be travelling on the highway
2) Linate - the city airport, accessible by public bus no. 73 leaving from Piazza San Babila in town
3) Orio al Serio - it's a bit of a stretch calling this a Milan airport, although all the budget airlines sell it as such. Located in Bergamo, the shuttle bus from the airport to Milan Central Station takes between 45 minutes to an hour (it once took me 1 1/2 hours!).

# Train
The Malpensa Express departs from Terminal 1 for Milan's Cadorna Rail Station every 30 minutes. It also stops at Milan Bovisa and Saronno stations. First train departs Malpensa at 06.45, the last train departs 21.45. Journey time is around 40 minutes.
Fare: EUR 9.00 one-way. Same day return is EUR 12.00, open return EUR 14.50.
The Malpensa Express information line is 02 20222.
There are also rail links from Malpensa airport to Milan Central and Milan Garibaldi Stations via Gallarate. A local shuttle bus connects the airport with Gallarate Rail Station every hour.
Shuttle bus fare: From Terminal 1 EUR 1.30, Terminal 2 EUR 1.05.
To Milan Central trains depart Gallarate from 05.54 until 23.24.
To Milan Garibaldi trains depart Gallarate from 05.30 until 23.00.

# Bus/Van
Bus services leave from outside the Terminal 1 Arrivals area. They serve various destinations in Milan and beyond. Journey times vary. There is also a local service to Gallarate Rail Station.
To Milan Central Station the Malpensa Shuttle departs every twenty minutes from 06.20 to 00.15. Journey time is around an hour.
Fare: EUR 4.50.
To Milan Gallarate Station the service runs every twenty minutes from 05.35 to 19.50.
Fare: From Terminal 1 EUR 1.30, Terminal 2 EUR 1.05.
Linate Airport Shuttle: There are 11 services daily between Milan's Malpensa and Linate airports. First bus departs Malpensa 07.50; last bus departs 00.25.
Fare: EUR 8.00.
There are also bus links from Malpensa airport to Turin, Genoa, Borgmanero, Novara, Como, Varese and Pavia.

My sister and I flew Ryan Air Stanstead to Bergamo, which is a popular airline with the budget jetsetter brigade! It is patronised by an interesting cross section of the public, appreciated by those who are fond of international travel but reluctant to spend £400 on a BA flight for the privilege of hopping across the channel and back.
The one-hour coach ride from Bergamo airport to central Milan was one colossal sprawl of businesses, factories and Mammouth style out-of-town shopping districts. A low lying thick fog shrouded us and our new surroundings in mystery as we left Orio Al Serio airport, and arrived through the city's main arteries into the centro città - perhaps unsurprisingly for a business centre in Northern Italy - dead as the night for a Friday evening!
We were immediately struck by the towering office blocks and neon signs of consumerist activity squaring up to the meglomaniacal grandeur of Central station and piazza.

Ryanair's Milan airport (actually in Bergamo) is one of their hubs, meaning cheap flights to the region from all over Europe. Milan's two main airports, Linate and Malpensa, are a lot closer, and are served by practically all of the major airlines, so you will most likely be able to book a flight to the city no matter where in the world you come from.

Whenever is your flight from Bergamo airport, there will be a bus departing accordingly, around 3 hrs before the flight departure time from Milan Central Station bus terminal to Bergamo airport. The one way journey takes approxiamate 45 mins and it costs 6 euros.
