Flying to Italy from the U.S. is not cheap, but there are some excellent deals to be found. You'll have a pretty good chance of getting a better-priced ticket if you fly to the bigger airports like the ones in Rome and Milan, and you also might get a direct flight. Having said that, shop around before making your final purchase.

When arriving in Milan into the Orio al Serio airport (probably flying with Ryanair) you have a cheaper option to get to downtown Milan. Instead of taking the Bus to Milan, catch a bus to the Bergamo station. It leaves just in front of the Arrivals and takes you there in 10 minutes. Then catch one of the frequent trains to Milano Centrale and in 50/60 minutes you're right in the center of teh city with a cost of about 6/7€, far less than the fare for teh direct bus (which will probably take longer to arrive too)

If you come from any further destination you'll probably be flying to Malpensa International Airport. Just have in mind that the airport is located about 60 kilometers away from the city so it will take you about an hour to get there.

Arriving at Milan's main airport, Malpensa is a breeze. It's located about twenty miles outside the city center so you take the Malpensa Express Train. It's located on the ground level, there are plenty of signs so don't worry about finding it. Go to the office to buy your ticket before you board. It's a very clean, smooth ride into the city which puts you at the Cadrona Station. From there there are plenty of taxis or the Metro to take you to wherever it is you need to go. If you are taking the Eurorail somewhere else you need to take the Metro line to Stazione Centrale which is where all the Eurorails go through.
The underground Metro is easy to understand and use and goes many convenient places. Buying a ticket from a ticket machine is hard to understand so buy it at the newstand and remember that you can transfer trains but a transfer only is good for 85 minutes then you have to buy a new one. The bus line is effiecient as well. You have to buy tickets in advance for the bus, also from newstands because the driver does not sell bus tickets and will let you know repeatedly if you step on without a pass.

If you are fling budget i.e RyanAir you arrive at Bergamo. If you decide to go staright to Milan (personally I would stay a night and look around as the high city is beautiful) there is a bus which leaves from outside the airport (Orio del Serio) It costs about 6€ and can be bought inside the airport.
If your flight is delayed, not that Ryanair would ever be late (!) Dont panic! My flight arrived at about 11.30pm more than one hour late and I was delighted to discover that the bus waits!
Getting around Milan is easy. There are so many ways, buses, metro and tram and all on the same type of ticket (which can be bought from machines at the m stations or tabacchi shops, signalled with a T)! I personally prefer walking round the historical centre but do try to get a tram as its like stepping back in time.

If you came to Milano by plane, try to land at Linate. Linate is 3Km from the town (10Km from the center), Malpensa is 40Km away.
Linate is a small airport and you'll be able to get out of it in few minutes, Malpensa is a very big one, so you'll probably have to walk a lot before being able to exit.
And the Taxi from Linate will cost you around 15/20 Euros, from Malpensa around 40/50 Euros.

More flying tips. If you can, try to fly into Linate as it is closest to the city centre. On arrival there are two buses you can take. As you walk out of the airport almost straight in front of you there should be an orange bus which costs 1€ and takes you into San Babila which is about 2 minutes from the Duomo. Alternatively get the blue STAM bus which costs 2€ and takes you to the central station. Easy Peasy! Alternatively you cold queue for hours for a taxi which would cost about 20€ Personally if you are visiting places or going to hotels I would get the bus into town and then get a taxi as the queues outside Linate for taxis are always horrendous...

I arrived at Milan Malpensa airport where the possibilities are train taxi or bus. My recommendation is to take the bus as it costs €4 and takes 50 minutes, the train costs double that but takes 40 minutes and both takes you to the stazione centrale.
Milan is easy to get to from anywhere as it is a commercial centre, my recommendation is to shop around as there are some good deals about at the moment (try Buzzaway if you are flying from Britain). Another alternative would be to fly into one of the smaller airports which will cost less and then catch a train.

By plane, 2 solutions: 'Linate' airport in the est of Milan, definitly the easiest way, just outside the city (5km from the center). You can take a taxi.
And the second one 'The international airport', good luck, 40km in the north-west of Milan.

I got there by Airplane form Munich. I landed at Malsapenta. If you can, try to arrage landing at the other aport, Linate, since it's much closer and CHEAPER by taxi!
I walk a lot, but that's because all the places us University studetns go to, is near by.
Otherwise go by metro and tram. But make sure to stamp your ticket. The truth is, they hardly have inspectors come and check to see who has one, but the first day i was about to go on without a ticket, there was an inspector...so i took it as a sign!
