    In the most touristic places like Gran Via, Puerta del Sol, Metro stations, El Rastro fleamarket... you must be careful with your purse, wallet and so.
    They can tell instantly if you are a tourist...

    I'm a long term resident in Madrid and while I haven't had any pickpocket experiences (mostly following the advice in many posts above) I will say to be absolutely vigilant in internet cafés (locutorios), not from objects being stolen but your data. I rarely use them now, and never to access any account after finding keyloggers open and tracking every keystroke. 3 times on the long street Calle General Ricardos and all were non-Spanish run (indian, bangladeshi). On one occasion I quickly screenshot everything and put the evidence on the pendrive, and cleared every scrap of info they had from other users, mostly gmail account details, and informed the owner I was going directly to the local police, only for the shouting to begin. 'You did this, you installed it!!', which quickly turned into pleading when I held up the pendrive with images. It's in police hands now. DO NOT USE LOCUTORIOS for anything but reading the news.
