From the USA there are several direct flights every day from Miami to Santa Cruz (American Airlines and LAB Bolivian Airlines) and two flights to La Paz. To connect to other Bolivian cities you are probably better of flying through Santa Cruz (from here you can basically fly anywhere in Bolivia and to the neighboring countries) since it is the main airport and you won't suffer because of the altitude. If you flight with LAB Airlines you can get a free ticket to any destination in Bolivia where this airline flies to (one stop over from Santa Cruz). If you are in a neighboring country (Peru, Brazil, Argentina, Chile, Paraguay) you can fly directly to Santa Cruz (there is also direct flights to La Paz if you are coming from Peru).
