it's difficult to travel here without knowing some rudimentary Portuguese (even Spanish helps a lot! for some reason most Brasilian/Portuguese people can understand Spanish, but not the other way around). My big regret is that I didn't learn enough; it would have really helped out! Not so many people speak English, but sign language helped

Many people do speak English in Sao Paulo, but please make an effort to learn some useful phrases in Portuguese. Don't be afraid to try, and you'll be pleasantly suprised at the patient and helpful response you'll get. Afterall, you're visiting their country, the least you can do is make an effort to learn their language :)This goes for any place you visit. I think it is important, not only to you learn something new, you also show respect for the country you're in. Happy traveling!

A wise suggestion: learn just enough Portuguese to be able to ask for prices and food. If you can speak Spanish, you will most likely be able to muddle through communications with people, and they’ll probably have fun giggling at your botched attempts. Well at least you’re trying, right?

Learn Portugese before you go (or at least as much as you can). Most Brazilians seem to understand at least some Spanish (forget English). Great place to learn another language and get to know truely nice people.
