The most upscale shopping center is El Tesoro which is located in El Poblado barrio. It comes with upscale stores, palm tree garden, waterfalls, even a miniature railroad. If you are lucky, you will witness a tango peformance. If you have the money and have an urge to buy clothes in Medellin, this is the place to go.
Other major shopping centers are San Diego nearby (which is quite a bit lower scale) and Unicentro in Laureles (which is also a lot more humble).
